I can see the REGISTER requests in my freeswitch host from Zoiper, but they never include the authenication details, so registration always fails with Unauthorized.

Hello,

The credentials will be sent after the server has challenged Zoiper. The first register will never contain the credentials.

Zoiper never sends the credentials. I can see the registration process with my service provider works that way, but not Zoiper.

Could you send us a debug-log file to support [at] zoiper [ dot ] com, with a link to this thread, for investigation?

You can find information how to create the debug-log file here: https://www.zoiper.com/en/support/home/article/176/Log_files_from_Zoiper_5

When you enable the debug-logger in Zoiper, simply restart the application and try to register your account. Then send the last generated file for investigation.
